Job summary

B&P Solutions, LLC is seeking a on-site Behavioral Health Case Manager to provide non-clinical case management for members with behavioral health and substance use disorders in North Carolina. You will coordinate care across veterans' healthcare systems, community resources, and local providers.

Role requires 3+ years post-graduate social work experience with advanced degrees (or 5+ with a BA). On-site work in Apex/Climax, NC, collaborating with teams and partners to ensure culturally

Qualifications

• Knowledge of federal and state mental health laws and patient confidentiality.
• Knowledge of HIPAA and Privacy Acts in civilian and military settings.
• Strong organizational and written/verbal communication skills.
• Experience with a Computerized Medical database system.
• Experience working on a virtual team.
• Current certification or licensure in evidence-based treatment for psychological trauma.

Responsibilities

• Provide non-clinical case management for eligible members with behavioral health and substance use disorders, coordinating across complex healthcare systems.
• Coordinate care using registries, health IT, and information exchange to ensure appropriate services are delivered.
• Link eligible members to housing, finance, food, shelters, and utility assistance resources.
• Refer crisis situations involving mentally unstable or high-risk individuals for timely intervention.
• Attend events to build and vet community partnerships and identify new resources.
• Provide behavioral health briefings at Yellow Ribbon and related events.
• Ensure proper signing and handling of consent forms.
• Prepare case management and critical cases; submit to COR monthly.
