Qualifications

• Bachelor's or Master's degree in a behavioral health science.
• 3+ years post-graduate experience in a social work setting if Master's Degree is held; 1 year within military setting.
• 5 years of post-graduate experience in a social work setting if Bachelor's Degree is held; 1 year within military setting.
• Knowledge of federal and state legal requirements pertaining to mental health counseling and patient confidentiality.
• HIPAA and Privacy Acts in civilian and military settings and ethics in mental health career field.

Responsibilities

• Provide non-clinical case management for eligible members with behavioral health and substance use disorders across healthcare systems.
• Provide care to eligible members using registries and health information exchanges.
• Coordinate non-clinical resource referrals for housing, finances, food, shelters, etc.
• Refer crisis situations with mentally unstable or high-risk individuals.
• Attend events to engage community partnerships and learn about new resources.
• Provide behavioral health briefings at Yellow Ribbon and similar events.
• Ensure signing/handling of consent forms.
• Prepare case management and critical cases; submit to COR monthly.
