Qualifikationen

• Bachelor’s degree in behavioral health, social work or related field is required.
• Associate’s degree plus 2 years related experience can substitute for a bachelor’s.
• HS diploma with 4+ years of behavioral health experience is also acceptable.

Aufgaben

• Observe members, document concerns and record observations in clinical records.
• Monitor health and safety of members, report concerns per policy.
• Develop and update individual service plans with time-specific actions.
• Maintain member records and treatment plans; complete progress notes with billing codes.
• Coordinate with primary care and interdisciplinary teams to improve quality of care.
• Follow policies on privacy, attendance and required trainings.

Ausbildung

Bachelor’s degree in behavioral health or related field

Associate’s degree in behavioral health or related field AND 2 years related experience

High School diploma with 4+ years behavioral health experience
