SGS Consulting is seeking a qualified health services professional in Columbus, OH to coordinate and monitor care plans for members. You will collaborate with case managers, medical directors, and family teams to improve outcomes and overcome barriers to care.

The role involves coaching, motivational interviewing, and ensuring member engagement while supporting access to needed services. Travel within the region up to 50% of the time is required.

Qualifications

• Bachelor's degree or non-licensed master level clinician required in behavioral health, human services, health services, or public health.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with Ohio delivery systems and child/family services.

Responsabilités

• Coordinates and implements care plan activities and monitors progress.
• Consults with case managers and supervisors to enhance quality of care.
• Collaborates with Child and Family Teams to support members.
• Identifies and escalates care quality issues through established channels.
• Negotiates options and services to meet members' needs.
• Uses motivational interviewing to promote healthy lifestyle changes.
• Provides coaching to empower informed health decisions.
• Facilitates active participation with providers in decision-making.
• Acts as a single contact to remediate gaps in care and access.
• Monitors, evaluates, and documents care following regulatory guidelines.
