SGS Consulting is seeking a licensed behavioral health case manager to assess, plan, implement and coordinate care for members, ensuring medical and behavioral health needs are met and promoting overall wellness.

You will use clinical tools to assess, coordinate with interdisciplinary teams, perform crisis intervention when needed, and help navigate benefits and programs to close gaps in care.

Qualifications

• Master’s degree in Behavioral/Mental Health or related field.
• Unencumbered Behavioral Health license in Illinois.
• Within 6 months, complete Behavioral Health Case Manager certification training.

Responsabilités

• Assess, plan, implement, and coordinate care management activities.
• Develop proactive actions to address issues and improve outcomes.
• Review data to identify member needs and care gaps.
• Provide crisis intervention and follow-up as needed.
• Act as single point of contact to remediate gaps in care.
• Collaborate with interdisciplinary care teams and attend case conferences.
