Job summary

SGS Consulting is seeking a licensed or non-licensed master level clinician with a behavioral health or human services background to provide care management, care coordination, and health system navigation. The role focuses on assessing needs, coordinating wellness plans, and improving quality of care across a diverse member population.

Qualifications

• 6+ months experience in Foster Care/juvenile justice, Adoption Assistance, Behavioral Health Services, trauma-informed care, ACEs, crisis intervention, or related practices.
• 2+ years experience in behavioral health, social services, psychology, sociology, or counseling.
• 1 year experience with MS Office Suite (Word, Excel).
• Willing to travel 10–15% to meet members regionally.

Responsibilities

• Using care management assessments to recommend approaches and evaluate benefit plans and programs.
• Identify high risk factors and implement proactive support interventions.
• Coordinate and monitor Wellness care plan activities and monitor member care needs.
• Consult with case managers and Medical Directors to improve quality of care and outcomes.
• Identify and escalate quality of care issues through established channels.
