Job summary

Woodhull Medical and Mental Health Center, part of NYC Health + Hospitals, is seeking a Nurse Aide to assist patients with activities of daily living and personal care under supervision, contributing to the treatment team in Behavioral Health.

You will observe and report changes, help plan daily activities, assist with meals and hygiene, and support routine care tasks, while following safety protocols and hospital policies to protect patient health.

Qualifications

• High school diploma or its educational equivalent.
• Successful completion of NYC Health + Hospitals training program in restraints and seclusion.
• Training in performance of vital signs and grooming within ninety (90) days of appointment.
• Valid and current BLS certification from the American Heart Association within sixty (60) days of appointment.

Responsibilities

• Observes patient's physical condition, mental state and social behavior and reports to team leader or Nurse-in-charge and provides individual nursing care for patients who are acutely ill, either physically or emotionally.
• Participates in planning and implementing an orientation program for new patients and gathers intake information and referrals.
• Plans daily living activities on the ward and contributes to a therapeutic ward environment.
• Assists in planning and implementing therapeutic activities with groups of psychiatric patients and encourages client involvement.
• Assists with basic patient care including bathing, hygiene, mobility, mouth care and related tasks.
• Assists patients with meals, serves trays, distributes water and nourishment.
• Assists with restraints/seclusion as trained, ensuring safety under supervision.
• Escorts patients to required locations as directed by staff within and outside the facility.
• Takes patient vital signs and records data in the EMR, including finger stick tests.
• Changes linen, keeps wards and storage areas neat and orderly.
• Disinfects personal items as required.
• May perform Nurse’s Aide tasks and related work as assigned.

NYC Health + Hospitals training: vital signs and grooming (90 days)

BLS certification (American Heart Association)
