Job summary

Turning Point Community Programs is seeking a direct service professional to assist members in achieving personal goals while living in the community. The role includes advocacy, medication management support, housing assistance, and after-hours on-call rotation to meet program needs.

Responsibilities include caseload management, documentation, and coordination with doctors, pharmacies, and community agencies. A valid CA driver’s license is required along with reliable transportation.

Qualifications

• High school diploma or GED required.
• 2+ years of direct mental health services experience preferred.
• Lived experience is a plus.

Responsibilities

• Assist members to identify and progress toward goals.
• Empower members in meeting goals; provide vocational support.
• Assist staff and members in activities, programming and groups.
• Facilitate communication and cooperation among agencies.
• Participate in client medication outreach; coordinate with doctor and pharmacy.
• Educate community and other agencies.
• Transport members to appointments and activities; field visits/medication outreach.
• Provide crisis intervention and on-the-spot support consistent with program philosophy.
• Carry a caseload depending on acuity and contracts.
• Maintain all client records and required documentation.
• Assist clients with accessing and maintaining housing.
• Attend to safety, health, and well-being of members.
• Is personally responsible for work hours and time management.
