Qualifications

Responsibilities

• Assist members in identifying and progressing toward goals.
• Empower members and provide vocational support.
• Support staff and members in activities, programming, and groups.
• Facilitate communication among agencies.
• Participate in client medication outreach; coordinate with doctor and pharmacy.
• Educate the community and other agencies.
• Transport members to appointments and activities as needed.
• Respond promptly to crises and provide on-the-spot support.
• Manage a caseload based on acuity and contract requirements.
• Maintain and document client records per standards.
• Assist clients with housing access and maintenance.
• Ensure safety, health, and well-being of members.
• Be accountable for work hours and time management.
