Responsibilities

• Assist members in identifying and progressing toward their personal goals.
• Provide vocational support and activity programming for members.
• Coordinate with staff and agencies to support member needs; advocate on their behalf.
• Facilitate communication among doctor, client, and pharmacy for medication outreach.
• Drive agency vehicle for transportation to appointments and activities as needed.
• Maintain client records, progress notes, and required documentation.
