Job summary

Meadows Behavioral Health is seeking an On-Call Behavioral Health Associate in Sunnyvale, CA to join a multidisciplinary treatment team. The role involves monitoring patient behaviors, providing continuous care, responding in crises, and recording data in patient records.

The successful candidate will have at least a high school diploma with preferred bachelor’s in behavioral science and about one year of clinical experience.

Qualifications

• Bachelor’s degree in behavioral science or related field preferred.
• High School Diploma/GED required.
• One year experience working in a behavioral health care facility preferred.

Responsibilities

• Ensures patient well‑being by maintaining a safe, structured environment and supervising patients.
• Interacts with patients, notes observations, and informs the group therapist and supervisor.
• Intervenes in behavioral crises using approved methods and provides crisis support.
• Provides education under supervision of Clinical Director and participates in care planning.
• Assists in admission and discharge processes and supports chart audits.
• Engages patients in activities to promote treatment goals and coordinates with the team.
• Documents patient records timely, accurately, and appropriately.
• Communicates with referents, other health care providers, and family members for care coordination.
• Supports data collection for outcomes and satisfaction surveys.
