This Position Requires a High School/GED Diploma

ESSENTIAL FUNCTIONS:

1. Ensure the well-being of patients and provide a positive, supportive, and structured environment.
2. Conduct safety checks and ensure supervision at 30-minute intervals or as per individualized guidelines.
3. Document clinical information accurately and timely in patient's medical records.
4. Assist in providing a safe, secure, and comfortable environment for patients, significant others, and staff.
5. Interact routinely with patients, observe behaviors, and communicate significant observations to nursing staff.
6. Assist patients with activities of daily living, including toileting, bathing, dressing, grooming, oral hygiene, meals, hydration, and changing bed linens.
7. Obtain patient vital signs, height, and weight as assigned, and document in patient records.
8. Facilitate patient educational and rehabilitative groups on topics such as social skills, coping skills, anger management, and independent living skills.
9. Engage patients in activities designed to encourage achievement of treatment goals.
10. Complete and maintain required documentation; assist with follow-up and incident reports as needed.
11. Provide or coordinate transportation for patients as required.
