Job summary

Health Connect America, Inc. seeks an Assessor at Georgia HOPE to conduct comprehensive behavioral health assessments, develop person-centered treatment plans, and support intake and authorization processes.

You will work with children, adolescents, and adults to determine appropriate care and create clear pathways to recovery. The role emphasizes collaboration with clinical teams and community partners, maintaining accurate documentation, and ensuring compliance with state requirements and COA

Qualifications

• Master's degree in Social Work, Counseling, or a related field is required.
• Must be eligible for licensure as LPC, LMFT, or LCSW in GA; full licensure preferred.
• Experience with crisis response and community-based services is preferred.

Responsibilities

• Conduct comprehensive behavioral health assessments and reassessments within required timeframes.
• Develop individualized treatment plans and collaborate with clinical teams.
• Coordinate with intake, service authorization, and documentation processes.
