Job summary

Georgia HOPE, part of the Health Connect America family, is seeking an Assessor to support children, families, and adults with behavioral health needs in Georgia. You will conduct assessments, help develop treatment plans, and coordinate with clinical teams to ensure appropriate care.

You will work within home, school, and community settings, building IRPs and documenting progress in the EHR, while aligning with COA and HIPAA requirements.

Qualifications

• Master's Degree in Social Work, Counseling, or a related field is required.
• Must be eligible for licensure as a LPC, LMFT, or LCSW in GA; full licensure preferred.
• Experience with behavioral health assessments and treatment planning is preferred.

Responsibilities

• Conduct comprehensive behavioral health assessments and reassessments.
• Develop person-centered treatment plans and IRPs.
• Coordinate with clinical teams and community partners.
• Ensure documentation in the EHR is accurate and timely.
• Support service authorizations and ensure regulatory compliance.
