Georgia HOPE, part of Health Connect America, is seeking an Assessor to support children, adolescents, and adults in accessing behavioral health services. You’ll conduct comprehensive assessments, develop treatment plans, and collaborate with clinical teams to ensure appropriate care and a clear path toward recovery.

You’ll work across home, school, and community settings, coordinating with families and community partners, documenting in the EHR, and ensuring regulatory compliance and timely

Qualifications

• Master’s Degree in Social Work, Counseling, or a related field is required.
• Must be eligible for LPC/LMFT/LCSW licensure in GA; full licensure preferred.
• State licensure eligibility for multiple states as listed in the posting.

Responsabilités

• Conduct comprehensive behavioral health assessments and reassessments within required timeframes.
• Develop person-centered IRPs and Treatment Plans with individuals and families.
• Coordinate referrals and connect with internal and external services and resources.
• Prepare accurate assessments, treatment plans, and documentation in the EHR.
• Support service authorizations and ensure regulatory compliance.
