Job summary

Georgia HOPE, part of Health Connect America, is seeking an Assessor to work with children, adolescents, and adults to access behavioral health services. The role emphasizes comprehensive assessments, person-centered planning, and collaboration with clinical teams and community partners.

You will develop treatment plans, support service authorization, document in the EHR, and coordinate with families to promote recovery and well-being in a community-based setting.

Qualifications

• Master's degree in Social Work, Counseling, or a related field required.
• Must be eligible for licensure as LPC, LMFT, or LCSW in GA.
• Experience with behavioral health assessment and treatment planning preferred.

Responsibilities

• Conduct comprehensive behavioral health assessments and reassessments.
• Develop individualized treatment plans.
• Support intake and service authorization processes.
• Document accurately in the Electronic Health Record (EHR).
• Coordinate referrals and connect individuals and families with resources.
• Collaborate with clinical teams, therapists, and families.
• Ensure compliance with COA, HIPAA, and payer guidelines.
• Respond to crisis situations and coordinate with crisis services.
