Job summary

Cape Cod Hospital in Massachusetts is seeking a Psychiatric Care Coordinator to support the Emergency Department and Behavioral Health services. You will coordinate timely treatment, disposition planning, and communication with patients and families.

Key duties include organizing admission and bed searches, coordinating with behavioral health teams, arranging transportation, updating crisis and safety plans, and documenting encounters in the electronic health record.

Qualifications

• Bachelor's degree in human services required; Master’s preferred.
• 1–3 years’ experience in mental health or co-occurring disorders.
• Experience with electronic health record documentation encouraged.
• Proficiency in Microsoft Office; strong organization and time management.
• Crisis Prevention Intervention Training within 1 month of hire; current BLS certification.

Responsibilities

• Respond to minimize ED time and boarding; support disposition planning.
• Take behavioral health histories and collect psychosocial information.
• Identify health-related social needs and coordinate services.
• Assist with bed searches and admissions to inpatient or partial programs.
• Coordinate transportation and transfers; update care plans.
• Maintain documentation and update crisis plans; liaison with providers.
