Job summary

Partnership HealthPlan of California is seeking a dedicated team member to coordinate medically necessary behavioral health services for adults and children. You will screen, assess, and connect members with appropriate providers while collaborating with a multidisciplinary team to promote quality outcomes.

The role requires strong communication, problem-solving, and organizational skills, plus the ability to manage multiple priorities in a fast-paced environment.

Qualifications

• Two years of experience in delivering care and services addressing behavioral health needs of individuals from diverse populations.
• Experience using evidence-based principles to address mental health and substance use disorders across multiple care settings.
• Ability to actively listen and engage members using motivational interviewing techniques.
• Valid California driver’s license and proof of automobile insurance as per Partnership policy.
• Bilingual skills in an approved threshold language are preferred.

Responsibilities

• Assists with a specialized caseload of members with behavioral health needs.
• Collaborates with care providers, members, and the multidisciplinary care team to address behavioral health needs.
• Acts as a support/resource to others within the organization for behavioral health cases.
• Receives calls from members seeking behavioral health services.
• Screens members using prescribed tools and directs to appropriate providers.
• Coordinate with county-based behavioral health services and other systems of care.
• Maintain documentation and case files in Partnership HealthPlan of California’s system.
