Job summary

Masonicare Home Health & Hospice in Wallingford, CT is seeking an experienced RN Hospice Nurse for weekend Baylor shifts. This part-time role covers 24–32 hours weekly, focused on delivering high-quality hospice care in patients' homes or assisted living settings.

The RN will develop and evaluate care plans, coordinate with other health professionals, supervise aides, and provide family education. NEW GRADS WILL NOT BE CONSIDERED; weekend scheduling options are available.

Qualifications

• Current RN license required.
• Hospice and/or home care experience preferred.
• New grads will not be considered.
• Weekend Baylor scheduling and part-time hours are available.

Responsibilities

• Develop, implement and evaluate the plan of care for hospice patients.
• Coordinate the plan of care with health professionals to ensure continuity of care.
• Supervise and educate Home Health Aides and LPNs as assigned.
• Provide education and support to patients' families regarding home hospice care.
• Provide supervision and support to clinical staff per regulatory guidelines.
