Hartford HealthCare at Home is growing and seeks a Baylor Homecare RN to provide skilled nursing in patients' homes. Shift options include Friday through Sunday or Saturday through Monday on a 24-hour schedule, serving the Mansfield, CT area.

Qualificações

• Active RN license in CT.
• Graduate of ADN or diploma program in nursing.
• NLN-approved BSN preferred.
• 1 year nursing experience; home care or medical/surgical preferred.

Job Description: Looking for Baylor Homecare RN For: either Friday, Saturday, Sunday or Saturday, Sunday, Monday 24 hour position to provide skilled services to our patients in the community.

As part of a person centered care model, this role will provide skilled nursing care to clients in home and community based settings ensuring the delivery of outcome based, high quality, cost effective care. This role will collaborate with all team members to achieve exceptional outcomes.
