Job summary

Sanctuary Personnel Limited in Northamptonshire is seeking a Band 6 Community Psychiatric Nurse to deliver high-quality mental health care in the community. • Ensure care plans are co-produced with service users and carers, and accessible to the multi-disciplinary team.
• Contribute to the Community Mental Health Service using evidence-based practices aligned with national standards.
• Act as a keyworker for service users with complex needs, sharing information appropriately.
• Provide structured therapeutic interventions through individual or group sessions.
• Plan and deliver patient care per the NMC Code of Practice or relevant standards.
• Drive continual improvements in clinical care standards within governance frameworks.

• Ensure that care plans are co-produced with service users and carers, maintaining relevance and accessibility for the multidisciplinary team.
• Contribute to the Community Mental Health Service using effective evidence-based practices aligned with national standards for clinical practice.
• Act as a keyworker for service users with complex needs, sharing sensitive information appropriately and in line with the Consent to Share Information Policy.
• Provide structured therapeutic interventions through individual or group sessions, adapting to the specific complexities of each client.
• Plan and deliver all aspects of patient care according to the NMC Code of Practice or relevant professional standards.
• Drive continual improvements in clinical care standards within the clinical governance framework to ensure the safest and highest quality of care is delivered.
