Sanctuary Personnel Limited in Northamptonshire is seeking a Band 6 Community Psychiatric Nurse to deliver high-quality mental health care in the community. The role offers a flexible mix of full-time or part-time hours and an hourly rate of £26.50, supporting your work-life balance while making a real impact on service users and carers.

Responsabilidades

• Perform independent assessments of service user care needs to develop, implement, and evaluate tailored care programmes.
• Ensure care plans are co-produced with service users and carers, and accessible to the multi-disciplinary team.
• Contribute to the Community Mental Health Service using evidence-based practices aligned with national standards.
• Act as a keyworker for service users with complex needs, sharing information appropriately.
• Provide structured therapeutic interventions through individual or group sessions.
• Plan and deliver patient care per the NMC Code of Practice or relevant standards.
• Drive continual improvements in clinical care standards within governance frameworks.
