Job summary

VitalCaring is seeking a Physical Therapy Assistant (PTA) to provide hands-on, in-home rehabilitation, helping patients regain strength, mobility and independence in their own environments. This field-based role emphasizes autonomy with support and a patient-first mindset.

As a PTA you will work one-on-one with patients, follow licensed therapist plans of care, document progress in the EMR, and collaborate with nurses and care teams to ensure coordinated, high-quality care that improves quality

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Current PTA license or state certification where required.
• 1+ year of home health experience preferred.
• Current CPR certification and valid driver's license.

Responsibilities

• Provide direct physical therapy services under the supervision of a licensed Physical Therapist.
• Follow individualized plans of care to improve mobility and reduce pain.
• Assist patients with exercises, gait training, balance, and functional mobility.
• Support transfers, strengthening, and activities of daily living.
• Document progress in the EMR and communicate with the care team.
