Job summary

Cornerstone VNA is seeking a Registered Nurse to join our home care team serving Milton, NH. This role offers autonomy, flexible scheduling, and meaningful one-on-one patient care across diverse home environments.

The RN must hold a NH or compact state license with at least 1 year of clinical nursing experience; home care experience is a plus. Collaboration with a multidisciplinary team is essential.

Qualifications

• Current NH or Maine RN license or other compact state license.
• Minimum 1 year of clinical nursing experience preferred (home care a plus).
• Excellent clinical assessment, communication, and organizational skills.
• Ability to work independently while being part of a collaborative care team.
• Comfort working in a variety of home environments.

Responsibilities

• Manage a diverse home care caseload with autonomy and flexibility.
• Perform comprehensive nursing assessments and develop patient-centered plans of care.
• Collaborate with physicians and interdisciplinary team members to coordinate high-quality care.
• Monitor patient progress, adjust care plans, and educate patients and families on health management.
• Complete OASIS assessments with accuracy.
• Document care accurately and timely during patient visits.
• Supervise LPNs and LNAs as needed.
• Participate in case conferences and discharge planning.
• Follow the Nurse Practice Act, agency policies, and home health regulations.
