Job summary

Cornerstone VNA is seeking a Registered Nurse (RN) to join our home care team serving the New Durham, NH area. This role offers autonomy, flexibility, and meaningful one-on-one patient care in patients' homes.

Base pay is $40.00–$43.00 per hour with mileage reimbursement and a travel expectation of ~95%. This is a full-time position with day shift options and flexible scheduling. We value compassionate, expert care in a nonprofit setting.

Qualifications

• Current NH or Maine RN license (compact state license accepted).
• Experience with home health nursing and OASIS is a plus.
• Excellent clinical assessment and communication skills.

Responsibilities

• Manage a diverse home care caseload with autonomy.
• Perform comprehensive nursing assessments and care plans.
• Collaborate with physicians and health team to coordinate care.
• Monitor patient progress and adjust plans; educate patients and families.
• Complete OASIS assessments accurately.
• Document care during visits; ensure timely records.
• Supervise LPNs/LNAs as needed.
• Participate in case conferences and discharge planning.

We're looking for a skilled and caring Registered Nurse (RN) to join our dedicated home care team serving the New Durham, NH and surrounding community.

What You'll Do:

Manage a diverse home care caseload with autonomy and flexibility

Perform comprehensive nursing assessments and develop patient-centered plans of care

Collaborate with physicians and interdisciplinary team members to coordinate high-quality care

Monitor patient progress, adjust care plans, and educate patients and families on health management

Complete OASIS assessments with accuracy

Document care accurately and timely during patient visits

Supervise Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) as needed

Participate in case conferences and discharge planning

Follow the Nurse Practice Act, agency policies, and home health regulations

What You Bring:

Current NH or Maine Registered Nurse (RN) license or other compact state license

Minimum 1 year of clinical nursing experience preferred (home care experience a plus)

Excellent clinical assessment, communication, and organizational skills

Ability to work independently while being part of a collaborative care team

Comfort working in a variety of home environments

Reliable transportation and valid driver's license (Mileage reimbursed!)
