Job summary

Interim HealthCare of Oklahoma City is seeking a Home Health RN Case Manager to shape care plans and provide hands-on nursing in patients' homes in OKC. You will work with a full care team to deliver attentive, patient-centered care.

You will collaborate with LPNs and therapists, perform visits and OASIS assessments, and advocate for autonomy that translates into real improvements in patient outcomes.

Qualifications

• Active RN license in Oklahoma.
• CPR certification.
• Home health experience is ideal, but we welcome strong clinical candidates from any background.
• Autonomy, strong assessment and critical thinking skills.

Responsibilities

• Develop and adjust the plan of care with the care team.
• Complete visits and OASIS assessments with a holistic patient view.
• Coordinate with LPNs, therapists and physicians to ensure continuity of care.
• Educate patients and families on diagnosis, medications and home safety.
• Advocate for patients to influence outcomes and care decisions.
