Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist for in-home and senior living care in Fox Point, WI. You will evaluate, plan, and deliver evidence-based therapy in patients' homes or communities, promoting independence and quality of life.

Qualifications

• Valid SLP license in the practicing state(s) or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.
• CCC (ASHA) certification is required or in process.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document care in FOX’s EMR system according to regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
