Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide in-home and senior living care in Janesville, WI. Build strong patient relationships and manage your own treatment schedule with optional telehealth components.

You’ll perform evaluations, create individualized plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life in older adults.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program
• Current CPR certification
• Current Certificate of Clinical Competence (ASHA CCC)

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
