Job summary

FOX Rehabilitation in Middletown, PA seeks a licensed Speech Language Pathologist to deliver one-on-one care in homes and Senior Living Communities. You’ll evaluate, treat, and document patient progress within the Geriatric House Calls model, enjoying autonomy over your schedule and meaningful patient outcomes.

The role supports telehealth and in-person visits, requires collaboration with an interdisciplinary team, and offers opportunities for professional growth within a clinician-led practice.

Qualifications

• Valid SLP license in PA or eligible to obtain
• Degree from an accredited Speech Language Pathology program
• Current CPR certification
• ASHA CCC credential
• Proficiency with EMR systems

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings
• Perform evaluations, develop care plans, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, longevity, mobility, and independence
• Document in FOX’s EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate
