FOX Rehabilitation is seeking a licensed Physical Therapist to provide in‑home, senior living, and virtual care to older adults in Emmaus, PA. You will deliver one‑on‑one therapy, conduct evaluations, and create individualized plans of care to improve mobility and independence.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsabilités

• Provide one‑on‑one PT to older adults at home or in communities.
• Perform evaluations and develop plans of care.
• Deliver therapy sessions to improve mobility and independence.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and fitness staff.
• Educate patients and caregivers to promote safety.
• Deliver telehealth when appropriate.
