Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one in-home and senior living care in Harrisburg, PA and surrounding areas. You’ll evaluate patients, create individualized care plans, and deliver evidence-based therapy to improve mobility and independence.

Role offers autonomy with flexible scheduling, potential telehealth components, and collaboration with an interdisciplinary team. New graduates welcome to apply. In-home care experience preferred.

Qualifications

• Must hold a valid Physical Therapy license in the practicing state(s) or be eligible to obtain one.
• Degree from an accredited Physical Therapy program.
• Current CPR certification and basic computer/EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence, and safety at home or in community settings.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
