Job summary

FOX Rehabilitation is expanding its in‑home Physical Therapy services in Emmaus, PA, seeking licensed PTs to deliver one‑on‑one care to older adults where they live. Therapists will assess mobility, create customized plans, and treat patients in private homes or senior living communities.

Clinicians work with an interdisciplinary team, document in FOX EMR, and may incorporate telehealth to extend reach and flexibility, while maintaining autonomy over patient care and scheduling.

Qualifications

• Licensed Physical Therapist with current PA license.
• Experience in geriatrics or home health care is preferred.
• Ability to work independently and manage your own schedule.
• Comfort with telehealth optional to extend reach.

Responsibilities

• Provide one-on-one physical therapy to older adults in private homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that improve mobility, independence, and quality of life.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, fitness staff).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
