Job summary

FOX Rehabilitation in Sutton Place, NY is seeking licensed Occupational Therapists to deliver one-on-one care to older adults in private residences and senior living communities. You’ll develop individualized plans, use telehealth when appropriate, and manage your own schedule to maximize patient outcomes.

Qualifications

• Valid Occupational Therapy license in the state of practice.
• Degree from an accredited Occupational Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one OT to older adults in private residences or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver sessions that support wellness, mobility and independence.
• Document effectively within FOX’s EMR system according to regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
