Job summary

Fox Rehabilitation is seeking licensed Occupational Therapists to deliver one-on-one care to older adults in private homes and senior living communities in Chantilly, VA. Our Geriatric House Calls model emphasizes autonomy, flexible schedules, and meaningful patient outcomes.

Clinicians may incorporate telehealth to extend access, collaborate with an interdisciplinary team, and support more patients while maintaining high-quality care and independence for clients.

Qualifications

• Current Occupational Therapy license in the state(s) of practice.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in homes or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility, independence, and quality of life.
• Document effectively in the EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
