Job summary

FOX Rehabilitation is growing in Prescott, AZ and seeks licensed Occupational Therapists to deliver one-on-one care to older adults in homes or senior living settings under the Geriatric House Calls model. You’ll have autonomy to schedule treatment, collaborate with an interdisciplinary team, and may incorporate telehealth to extend access and improve continuity of care.

Qualifications

• Must hold valid OT license or be eligible to obtain one.
• Experience conducting home- and community-based OT evaluations.
• Ability to develop patient-centered plans of care.
• CPR certification preferred.

Responsibilities

• Provide one-on-one occupational therapy to older adults in homes or senior living settings.
• Perform evaluations and develop individualized plans of care.
• Deliver evidence-based treatment to improve mobility and independence.
• Document care accurately in FOX EMR in line with regulations.
• Collaborate with an interdisciplinary team and educate caregivers.
