Banner Health is seeking an Occupational Therapist for a PRN Home Health role in Colorado. The position involves autonomous yet supportive work across sub-assigned territories with mileage reimbursement and a company cell phone/tablet for documentation in Home Care Home Base.

The role includes evaluating patients, planning and supervising OT treatment, coordinating with physicians and families, and ensuring regulatory compliance. Anticipated closing window is 2027-02-13.

Qualifications

• Must have completed an OT educational program approved by the governing board.
• Must possess a current OT license from the State of practice.
• BLS certification required.
• Ability to become proficient with computer software applications in a patient care environment.
• Arizona Fingerprint Clearance Card required for Banner Home Care AZ.

Responsibilities

• Evaluates patient's functional abilities and limitations and plans care.
• Implements and documents treatment plans in accordance with standards.
• Communicates results and progress to physicians, nurses, patients and families.
• Ensures department productivity and participates in process improvements.
• Supervises and delegates tasks to support personnel as applicable.
