Responsibilities

• Evaluates patient functional abilities and determines need for intervention; establishes care plan and goals.
• Implements and documents individualized treatment plans with patient and family involvement.
• Documents according to department and regulatory standards; maintains legible, timely records.
• Communicates results and patient progress to physicians, nurses, patient and family; coordinates with interdisciplinary team.
