Banner Health is seeking a Home Health Occupational Therapist in Greeley, Colorado. This PRN position offers autonomy with 1:1 patient care, mileage reimbursement, a company cell phone, and a laptop.

The team emphasizes community well-being and collaboration within an interdisciplinary group. Responsibilities include evaluation, planning and supervising therapy, documenting progress, and coordinating with physicians and caregivers to ensure optimal patient outcomes.

Formación

• Must have completed an OT program approved by the governing board.
• Current Occupational Therapy license in state of practice.
• BLS certification required.
• Ability to become proficient with computer software used in patient care environments.

Responsabilidades

• Evaluates patient’s functional abilities and establishes a plan of care and goals.
• Implements treatment and monitors progress; involves patient and family in decisions.
• Documents according to department and regulatory standards with timely, legible records.
• Communicates results and patient progress to physicians, nurses, and the care team.
