Job summary

Impact Health PC in Augusta, ME seeks an experienced Nurse Practitioner to provide comprehensive Medical/Primary Care to residents in skilled nursing facilities. You will assess, diagnose, and manage acute and chronic conditions, and develop individualized treatment plans.

You will collaborate with physicians, nursing staff, and interdisciplinary teams, order labs and imaging as needed, ensure timely documentation, and stay compliant with regulations.

Qualifications

• Active Nurse Practitioner license in Maine.
• Minimum 2 years of post-certification clinical experience preferred.
• Experience in skilled nursing facilities, primary care, hospitalist medicine, or related settings.
• Ability to work independently within scope of practice.
• Proficiency with EHRs and clinical technology.

Responsibilities

• Provide comprehensive Medical/Primary Care to residents within skilled nursing facilities.
• Assess, diagnose, and manage acute and chronic medical conditions.
• Develop individualized, evidence-based treatment plans.
• Order and review medications, laboratory tests, imaging, and other diagnostic studies.
• Complete timely and accurate clinical documentation and orders.
• Collaborate with physicians, nursing staff, specialists, and interdisciplinary care teams.
• Educate patients and families regarding medical conditions and treatment plans.
