Impact Health PC in Rocky Hill, CT seeks a Nurse Practitioner to provide comprehensive care in skilled nursing facilities. You will assess, diagnose, and manage acute and chronic conditions, develop treatment plans, and collaborate with physicians, staff, and interdisciplinary teams.

You’ll maintain timely documentation, educate patients and families, and ensure compliance with facility, state, and federal regulations while enjoying autonomy and strong clinical support with ongoing professional

Qualifications

• Active NP license required (Maine).
• Experience in SNF, hospitalist, ED, or ICU preferred.
• Strong communication and organizational skills.
• Ability to work independently and collaboratively with physicians and care teams.

Responsibilities

• Assess, diagnose, and manage acute and chronic conditions.
• Develop evidence-based treatment plans and complete timely clinical documentation.
• Place appropriate orders and coordinate with physicians, nursing staff, specialists, and care teams.
• Educate patients and families regarding conditions and treatment plans.
• Maintain compliance with facility, state, and federal regulations.
