Job summary

Northeast Healthcare Recruitment is seeking an experienced Nurse Practitioner or Physician Assistant to provide autonomous, continuous care in skilled nursing facilities. You will manage your own patient panel, conduct wellness visits, and coordinate with physicians to keep care seamless.

The role offers 6-hour shifts, no on-call, and productivity-based pay with potential of earning up to $624 per day, plus a CME stipend and comprehensive benefits.

Qualifications

• Board certification required (e.g., AGNP, FNP; PA-C) active and unrestricted.
• Valid, unrestricted NP or PA license in the state of operation.
• Master's degree in Nursing (NP) or accredited PA program required for credential.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.

Responsibilities

• Provide primary and episodic care to residents in skilled nursing facilities.
• Manage own patient panel and build long-term relationships with patients and families.
• Conduct annual wellness visits and H&Ps, manage chronic and acute conditions.
• Coordinate with collaborating physicians, specialists, and facility staff to ensure seamless care.
