Froedtert ThedaCare Health's Workforce Health team is seeking an experienced APRN-NP or PA to provide primary care in onsite employer clinics, manage your own patient panel, and contribute to a population-health focused model of care within a collaborative team.

Qualifications

• Deliver primary care services in onsite employer health clinics (must have Family Medicine experience working independently with all ages).
• Diagnose and treat acute and chronic conditions across the lifespan.
• Focus on preventive care, wellness, and population health outcomes.
• Collaborate with employers to support workforce health initiatives.
• Utilize evidence-based medicine to improve patient outcomes and engagement.
• Maintain continuity of care through relationship-focused practice.
