Job summary

Froedtert ThedaCare Health - Workforce Health in Manitowoc, WI is seeking an APRN- NP or PA to deliver primary care in onsite employer clinics. You will manage your own patient panel with autonomy and work within a collaborative care team.

This 0.9 FTE position offers a set schedule, no on-call responsibilities, and a focus on preventive care, wellness, and population health. You will be supported by an APP Medical Director and the resources of a large regional health system.

Qualifications

• Deliver primary care services in onsite employer health clinics (must have Family Medicine experience working independently with all ages).
• Diagnose and treat acute and chronic conditions across the lifespan.
• Focus on preventive care, wellness, and population health outcomes.
• Collaborate with employers to support workforce health initiatives.
• Utilize evidence-based medicine to improve patient outcomes and engagement.
• Maintain continuity of care through relationship-focused practice.

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing Workforce Health team. This is a unique opportunity to practice comprehensive primary care in an onsite employer-based setting while enjoying the support of a collaborative care team, engaged APP leadership, and the resources of an integrated health system.

As a provider on our team, you will have the opportunity to build meaningful patient relationships, manage your own patient panel, and practice with a high degree of autonomy while contributing to a population health-focused model of care.

Position Location

Manitowoc, Wisconsin

Manitowoc, located on Lake Michigan, is a city with a rich maritime heritage. It boasts beautiful parks, historic sites, and a welcoming community. The city offers easy access to outdoor activities, including boating, fishing, and hiking, as well as a variety of cultural events and festivals. Manitowoc is about an hour and a half from Milwaukee Mitchell International Airport and approximately 45 minutes from Austin Straubel International Airport in Green Bay, offering multiple travel options.

Current FTE and Schedule

• This is a 0.9 FTE APP position
• Mondays, Wednesdays, and Fridays 8:00 am – 4:30 pm
• Tuesdays and Thursdays 9:30 am – 5:00 pm
