Northeast Healthcare Recruitment, Inc. (NEHCR) seeks an experienced Nurse Practitioner or Physician Assistant to provide primary and episodic care in skilled nursing facilities with autonomous practice and a dedicated patient panel.

You will build lasting relationships with residents and families, manage rounds during 6-hour morning shifts, and chart from home with modern EHR tools and UpToDate access. The role offers strong earning potential and comprehensive benefits.

Qualifications

• Board certification and active/unrestricted license in the state of operation.
• Master's degree required for corresponding credential.
• SNF/long-term care experience preferred.
• Strong independent clinical judgment and ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients per day, including wellness visits and H&Ps.
• Manage chronic and co-morbid conditions and respond to acute changes.
• Review medications and orders and coordinate with collaborating physicians.
• Engage with families and participate in care conferences for seamless care.
