Job summary

Curana Health, Inc. is seeking a Nurse Practitioner or Physician Assistant to deliver proactive primary care to seniors across our communities. You will perform health assessments, create treatment plans, and manage chronic conditions with a focus on prevention and continuity of care.

You will work with physicians and care teams, maintain EMR documentation, and travel to assigned facilities to provide care that supports independence and quality of life for residents.

Qualifications

• Licensed Nurse Practitioner or Physician Assistant with current board certification.
• Master's in Nursing required for NP route; PA requires certification.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and verbal communication abilities.

Responsibilities
