Job summary

Curana Health is seeking Nurse Practitioners or Physician Assistants to lead high-quality primary care for seniors across our community network in the United States. The role emphasizes autonomous clinical leadership within skilled nursing, assisted living, and independent living settings.

You will perform comprehensive assessments, create treatment plans, manage chronic diseases, and collaborate with physicians and care teams to improve outcomes while enjoying a mission-driven, flexible work

Qualifications

• Active nurse practitioner or physician assistant license in state of operations.
• Master's degree in nursing or PA studies.
• Strong leadership, interpersonal and communication skills.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Conduct comprehensive health assessments and develop individualized treatment plans.
• Diagnose, treat, and manage acute and chronic conditions with a focus on preventative care.
• Maintain timely documentation in the EMR and ensure regulatory compliance.
• Collaborate with physicians, facility leadership, specialists, and families to coordinate care.
