Job summary

Curana Health is seeking Nurse Practitioners or Physician Assistants to deliver high-quality primary care to seniors in our communities. You will perform assessments, develop personalized treatment plans, and manage chronic conditions with a focus on prevention and wellness.

You'll build strong relationships with residents and families, collaborate with physicians and care teams, and utilize advanced technology to improve outcomes.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Master's Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.

Responsibilities

• Perform comprehensive health assessments and develop treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build Meaningful Patient Relationships with residents, families, and care teams.
• Collaborate Across the Care Continuum with physicians, clinical staff, and facility leadership to coordinate care plans.
• Maintain accurate EMR documentation and ensure regulatory/compliance standards.
