Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in skilled nursing and senior living facilities. You will provide direct patient care, focus on promoting wellness, managing chronic conditions, and coordinating with other healthcare providers.

This role requires travel to and from multiple facilities, with autonomy of practice and a strong support system.

Qualifications

• Licensed nurse practitioner or physician assistant in the operating state with current board certification.
• Master's degree in nursing or physician assistant studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication; team player.

Responsibilities

• Perform comprehensive patient assessments and develop care plans.
• Provide direct primary care in SNF and senior living facilities.
• Coordinate with clinicians, staff, and families for ongoing care.
• Educate residents and families on wellness and disease management.
• Document care in EMR and ensure regulatory compliance.
• Participate in quality improvement initiatives.
