Job summary

Advocate Health Memory Center is seeking an Advanced Practice Provider (NP/PA) to join our memory care and brain health team. You will diagnose, treat, and manage care for patients with Alzheimer’s disease and related memory disorders, collaborating with neurologists, geriatricians, and neuropsychologists.

The role offers autonomy in managing an outpatient clinic, participation in research, and caregiver education, with a 40-hour workweek adjacent to Advocate Lutheran General Hospital.

Qualifications

• Master’s Degree in Nursing required.
• APRN license with active board certification in area of practice.
• DEA registration and Illinois Controlled Substances license prior to hire.
• BLS and/or ACLS/PALS/NRP as required.
• Active RN/APRNP credentials in state of practice.

Responsibilities

• Assess, diagnose, and manage treatment plans for adults with dementia and cognitive disorders.
• Order, perform, and interpret diagnostic studies; prescribe therapies.
• Provide patient- and caregiver-education and coordination of care.
• Collaborate with a multidisciplinary team to implement individualized care plans.
• Participate in quality and research initiatives to advance dementia care.
• Maintain accurate EMR documentation and compliant coding.
