Job summary

Advocate Health Memory Center, adjacent to Advocate Lutheran General Hospital, seeks an Advanced Practice Provider (NP/PA) to join our dementia care team. You will deliver patient-centered care for individuals with Alzheimer’s and related disorders, working with a multidisciplinary group to create individualized treatment plans and support families.

Position offers 40 hours/week with opportunities for involvement in education and research initiatives within a nationally recognized memory care

Qualifications

• Active RN and APRN license in state of practice.
• Active national board certification in area of clinical practice.
• Active DEA registration prior to hire.
• Illinois Controlled Substance License prior to hire (if Illinois practice).
• Active BLS and/or ACLS, PALS, NRP as required by clinical practice.

Responsibilities

• Assess and manage treatment plans for dementia patients.
• Order, perform, and interpret diagnostic studies and therapies.
• Collaborate with a multidisciplinary care team to develop comprehensive plans.
• Maintain accurate and timely documentation in the EMR.
• Participate in quality, education, and research initiatives.
